The Process

  4. Completion of I-693 Form: After all tests and immunizations are complete, our physician will fill out the I-693 form, which will be sealed in an envelope to be submitted to USCIS.

Price


If you have everything(immunizations ETC) your Immigration Medical Exam Cost will be $350( includes Physical Exam,TB test, RPR, Gono test). If you do not have your Immunization records the cost would be $500( includes the above and all titres neccessary). If you need vaccination it would be recommended to you after the test you can get it at your local pharmacy or your primary care physician or you can get it with us if we have the vaccines available at that time.


If you need titers (Varicella, Hep-B, MMR) and immunizations(Tdap, Varicella, MMR, Hep-B,Flu vaccine,Covid Vaccine) that would be provided for an extra charge. Please ask staff for details.


Once we have your results, titers and immunizations we'll finish your form and give you a sealed envelope and as well as a copy for your records.

  • At your appointment, our Providers will get a detailed history and physical with necessary blood test (RPR, Gonorrhea) and a blood TB test

What should I bring in when I come in for appointment of Immigration medical examination

Valid Government Issued ID: Passport, US Driver’s License, State ID, Proof of prior vaccinations (if available), Copies of any previous chest X-rays, if any

Immigration Physical Check List

What do I need? 

  1. Have to do in USCIS Civil Surgeon Clinic : Physical+ RPR(syphilis blood test )+Tb test via blood Urine for gonorrhea 
  2. Proof of 
  • Tetanus shot  I don't have it ---> we will give you one in the office 
  • Measles, Mumps Rubella vaccine I don't have it ----->Check blood for immunity to see if you already had it, if you come non immune then you get a MMR vaccine 
  • Varicella (chicken pox )shot ----> I dont have it ---> Check blood sample and see if you a re immune to it , if not you get the vaccine 
  • Hepatitis B vaccine if you are less than 65 years old --I don't have it ----> we will give you the vaccine in the office OR check blood for previous immunity to see if you already had it.
  • Hepatitis A vaccine if you are less than 18 years old I don't have it ----> we will give you the vaccine in the office OR check blood for previous immunity 
  • Flu shots (from Oct to March )- get your proof to the office on your first appointment 
  • Covid vaccine Get your confirmation Or We will give it to you in the office.
